Occasionally a team member may be using a work method that affects quality. A customer service representative, for example, may be wearing the telephone headset improperly, making it difficult for the customer to comprehend what is said.
Or a construction worker may be adding excessive water to a concrete mix to make placement easier, unaware that the final strength of the concrete will be diminished.
